      are to be congratulated on producing a practical method to encourage aesthetic surgeons to base their pre-operative clinical assessments of patients on a history of living with the appearance problem that is causing concern. The trouble has always been that this symptomatology is a history of psychological distress and psychological dysfunction, which plastic surgeons have not been trained to understand and which they instinctively feel is outside their zone of comfort. Most aesthetic surgeons are quite content to continue their practices in ignorance of their patients' psychological motivations for surgery and, to an extent, patients collude because they are instinctively secretive about their self-consciousness for fear of being ridiculed.
