      Summary

Based on 11 years' experience, the best speech results have been obtained in patients with a cleft palate when the palate has first been closed with a minimal, atraumatic palatorrhaphy, followed six months later by the construction of a dynamic muscular sphincter at the site of the velopharyngeal incompetence.
In my opinion, this easy, quick, atraumatic surgery can solve the speech problem of the cleft palate patient if the palatorrhaphy is performed when the patient is 2 years old, and the dynamic sphincter is constructed six months later.
